Overview of torsade de pointes
- Contrary to dogma, QT prolongation may not be a good predictor of torsade de pointes. This arrhythmia requires both early afterdepolarizations (EADs) and spatial dispersion of repolarization across the left ventricular wall (ie, transmural heterogeneity in prolongation of the action potential).
- EADs provide the trigger, and increased dispersion of ventricular repolarization the substrate, for development of torsade de pointes.
